I am stucked in a situation when i keep the placeholder in the lookup ,i am unable to see the lookup values.

Could you, please, provide us with more detailed information on your case? Do we understand the problem right?
1. You added a placeholder to the lookup field.
2. After that what exactly takes place with the field?
- The field became not clickable at all.
- You click the field but nothing happens: neither drop-down list nor pop-up window is displayed by the system to provide you the opportunity to choose the lookup value
- You click the field and the blank pop-up window or drop-down list are displayed by system.
Please, describe, what exactly you did and what detailed result you got. Thank you.

If you can't see lookup values when you choose the lookup then it seems that this lookup field is not bound to the existing lookup. The lookup should look like this http://prntscr.com/lfijx1 (please pay attention to "bindTo" raw). In code provided by you there is no "bindTo" raw.
Please re-add the lookup on the page using page designer and after that insert the part of code that is responsible for placeholder and it will solve your issue.
