It's possible to add multiple input file into a page as seen on the attachment?
The user need to attach specific documents and the system must mark some checkbox associated to the document.
I will appreciate your help.

Dear Uriel,
'Attachments and Notes' detail is not customizable as this is a quiet specific detail. There is a problem registered for R&D team to make this detail more flexible. It was already accepted and will be implemented in one of the further releases.
