Feedback Rating Email

Hello Everyone, i have set up my CRM and all is working fine  although an older version 7.12.2, We require the feedback form with ratings integrated to send email on the cases reported and resolved to have an insight on team performance, although while implementing this the form is not providing the option to choose the level of satisfaction.Kindly assist.
